I understand that tattoo ink is not FDA-approved and accept the risk of a potential allergic reaction.
I have informed my tattoo artist of any conditions that could affect the healing of my tattoo. I confirm that I do not have any medical or skin conditions, including but not limited to acne, scarring, keloid, eczema, psoriasis, freckles, moles, or sunburn in the area to be tattooed, which may interfere with the tattoo process. If I have any type of infection or rash anywhere on my body, I will notify my tattoo artist.
I confirm that I am not pregnant or nursing and that I am not under the influence of alcohol or drugs.
I acknowledge that infection is a possible risk when receiving a tattoo, particularly if I do not properly care for it. I have received aftercare instructions and agree to follow them while my tattoo is healing. I understand that any touch-up work required due to my own negligence, including damage caused by sun or ultraviolet exposure, will be at my own expense.
I acknowledge that a tattoo is a permanent modification to my appearance and that no guarantees have been made regarding my ability to change or remove it. To the best of my knowledge, I do not have any physical, mental, or medical impairments that would affect my well-being as a result of receiving a tattoo.
If my tattoo includes writing in a language other than English, I understand that Swan And Sword Tattoo and its artists are not responsible for the meaning or translation of the text.
I accept that variations in color and design may occur between the chosen design and the final tattoo. I understand that if my skin tone is darker, the colors may not appear as vibrant as they do on lighter skin.
I affirm that receiving this tattoo is my own free will and choice, and I consent to the location of the tattoo and the tattooing procedure.
I agree to follow all instructions provided to me regarding maintaining a sanitary environment during the tattoo process.
I confirm that the tattoo design has been drawn to my specifications and agree to allow for the artist's interpretation of the image.
I grant Swan and Sword Tattoo and the Artist performing the tattoo permission to take photos/videos of my tattoo on social media platforms and websites for promotional purposes.
Finally, I agree to notify the artist immediately if I feel lightheaded, dizzy, or faint before, during, or after the procedure. Failure to do so releases Swan And Sword Tattoo and the artist from all responsibility.
